Membership applications can be submitted by downloading a printable application
(located on the right side of the screen) and mailing to:

Payment of membership fees must accompany all mailed applications. Checks payable to MDMHA. Memberships
are annual and run from January 1st - December 31st. Deadline for membership renewals is February 15, 2012. All
renewals received after this date will be assessed a $5 late fee. New memberships accepted anytime during the calendar
year.
